What would cause these symptoms? Pain and swelling after ejaculation, difficulty urinating, slow flow,? -

urgency to go and not feeling empty and sometimes not going at all

Well a couple of things could cause that. Obviously the first thing that comes to mind is a sexually transmitted infection, so if you have had unprotected sex recently, get yourself down to the clinic for a check up.
Otherwise, it could be that you have a urinary tract infection or possibly urethritis which is inflammation of the urethra. Both these conditions will clear up on their own given time, but a course of antibiotics is the usual remedy and provides a quick-fix..
I had the same symptoms with a prostate infection but it could also be a urinary tract infection or the clap. (Gonorrhea )With the clap one usually has a leakage of pus when it gets real bad.
I would call it something to make me go to a doctor, very quickly.
